4. The wage is determined only by the Division Manager and Personnel Department
5. Enter the day / week the job will work
6. Enter the hours / day the job will work
7. Estimated duration of job in days, weeks, etc.
8. Date the employees are to report to job site
9. Start Time
10. Check either the a.m. or p.m. box, not both
Make a seperate line entry for any Craft where the information in columns 3-10 is not the same.
