Unidentified Hazardous Conditions - When inspections are not through enough, or when they are performed too infrequently a hazardous condition can be entirely overlooked.

Uncorrected Hazardous Conditions - When an employee or a supervisor may know about a hazardous condition but not bother to do anything about it, or fail to follow up on the reported item.
