Hazardous Acts

Unaware - If employees are not aware that a situation or procedure is hazardous, they will not know enough to take proper precautions.

Unable - Due to lack of training, poorly designed or improper equipment, or personal physical limitations may prevent an employee from doing a job safely.

Unmotivated - They may believe the rules are unnecessary, and think the risk they are taking is small, and worth taking to save some time or effort. They choose not to exercise goal health and safety judgment.
